Leaving out providers or treatment dates
SSA needs accurate medical-source details to request records. Include every relevant source and review names, addresses and dates. A missing specialist or facility can leave an important period undocumented.
Listing diagnoses without functional detail
Explain what the condition prevents or limits in practical work terms. Describe sitting, standing, walking, lifting, reaching, concentration, pace, social interaction and attendance only where relevant, with concrete examples.
Using inconsistent dates
Make the alleged onset date, last day worked, treatment timeline and job history as accurate and consistent as possible. If you are uncertain, say so and explain rather than creating a precise date without support.
Missing follow-up requests
Open SSA mail, monitor the application and answer calls or forms promptly. Keep proof of delivery. Tell SSA when contact details, work, providers or medical conditions change.
Treating screening as an application
A questionnaire, benefit estimate or eligibility screen is not the government application. Confirm that you have actually filed through SSA and retain the official confirmation.
